Who owns the app listing?

You do. It is published under your practice's developer account, so the listing and its reviews stay yours.

What if Apple rejects the submission?

We handle the response and resubmission. That work is inside the scope, not billed as an extra.

Do we still need SMS?

Usually yes, for clients who have not installed the app. Push reduces SMS volume rather than eliminating it.
