Where this estimate is wrong

  • It assumes an unfilled slot generates no revenue. If your team reliably backfills cancellations, your real loss is lower.
  • It ignores the staff time spent booking, confirming and chasing the appointment that did not happen. Your real loss is therefore higher in a way this does not capture.
  • It uses your estimate of your no-show rate. Most practices that have never measured it estimate low.

The limit of a slider

A total is not a diagnosis

The figure above tells you how big the leak is. It cannot tell you which of the five stages in your appointment journey it is escaping through — and those five stages cost very different amounts to close.

  1. Someone decides they need you

    They find you outside business hours, or they find you and do not want to make a phone call. There is no record of them at all — which is why this leak is invisible.

  2. They try to book

    Booking is only possible while you are open, which is exactly when they are at work. Meanwhile the clinic down the road takes the booking at 9pm.

  3. The appointment waits

    Between booking and arrival, life happens. Without a confirmation, a 48-hour nudge and a 24-hour reminder, the decision to not show gets made in silence.

  4. They arrive — or they do not

    Payment after the service means a no-show costs everything and recovers nothing. A booking with no money attached is a reservation, not a commitment.

  5. They should come back

    The recall or follow-up depends on someone remembering to run a report and make calls. Nobody owns the number, so nobody notices it falling.

Method

How the number is worked out

How is the figure calculated?
Appointments per week × your no-show rate × 4.33 weeks per month × your average appointment value. That is the whole formula. 4.33 is used rather than 4 so the annual figure does not quietly understate itself.
Is this a prediction of what I would save?
No. It is an illustrative estimate of current loss. The scenarios show what the same arithmetic produces at a lower no-show rate — they are arithmetic, not a forecast, and no system is guaranteed to move your rate to any particular number.
I do not know my no-show rate.
Use the starting figure for your practice type and adjust it upward until it feels uncomfortable. Practices that have never measured the rate typically guess two to five points low, because a late cancellation rarely gets recorded as anything at all.
Should I include late cancellations?
Yes, if the slot went unfilled. A cancellation at 8am for a 9am appointment costs the same as a no-show. A cancellation two weeks out that was rebooked costs nothing and should be left out.
